2. Print and complete the downloaded Account Access Agreement Please use the "Print Button" located in the Adobe Acrobat toolbar
3. Mail your completed Agreement to:

Upon receipt of your completed and signed Account Access Agreement, your login information will follow via standard postal mail. Please allow 2-3 weeks for delivery.
If you provide a legible e-mail address, we will send you an advance copy of your login instructions.
